You will be prompted to enter agent information, upload AHIP and E&O. Once information is validated, you will be asked for a referral code. Enter referral code DP-000870. That code confirms that you are contracting with ISMHealth.net

Please select the carriers you would like to onboard with and continue entering the appropriate information requested.

Once you sign off on policies and procedures & hit “finish”- that sends us an alert that you would like to contract. Please alert me once you have completed.

 

Remember: this is our contracting request system & this does not contract you with the carrier. You will receive each carrier’s contract in 24-48 business hours.
